Dean Health Plan/SSM Health – Triangle Neighborhood Program
To help battle food insecurity, Dean Health Plan/SSM Health has partnered with Second Harvest to provide a mobile food pantry to the Triangle Neighborhood in Madison, WI. Volunteers staff the food pantry, which moves between community rooms in various neighborhood apartment complexes.
HealthPartners – Better Shelf for Better Health
HealthPartners has partnered with Valley Outreach Food Shelf to distribute healthy food to food-insecure populations. Each year the food shelf programs distribute more than 800,000 pounds of food to 1,500 households and 3,500 individuals
Kaiser Permanente – Hunger Free Colorado
Kaiser Permanente Colorado (KPCO) has partnered with Hunger free Colorado to identify eligible members for Supplemental Nutrition Assistance Program (SNAP) and the Women Infants and Children program. Once identified, KPCO helps members enroll and redirects them to food pantries, for group meals and other food resources.
Presbyterian Healthcare Services – La Cosecha
Presbyterian Healthcare Services has partnered with the La Cosecha Community Supported Agriculture program to support access to healthy, affordable food. The partners identify low-income, food-insecure families in the South Valley and International District and connect them with weekly bags of fresh, locally grown produce.
UCare – Twin Cities Mobile Market
The Twin Cities, MN metro area has been named the fifth-largest urban food desert in the U.S. To help combat widespread food insecurity, UCare has partnered with the Amherst H. Wilder Foundation to launch the Twin Cities Mobile Market to sell healthy foods at below-market prices in low-income neighborhoods.

GHC-SCW has established a Health Equity Committee to help reach health equity in Madison, WI. As part of their commitment to addressing institutional biases, the Committee partnered with YWCA-Madison to provide GHC-SCW staff with equity training and have partnered with Second Harvest to provide food insecurity support for the public.
HealthPartners – Community Health Worker: Medicaid Population
HealthPartners has launched a pilot to assess the impact of the community health workers (CHWs) within a targeted population. The health plan hopes CHWs can assist at-risk populations in managing their health, including bridging language and cultural barriers.
UCare – Somali Community Resettlement Program
To support recently located refugee populations, UCare Foundation has partnered with the Somali Community Resettlement Program. The program aims to improve the overall health of refugees and reduce the health disparity gap in the Rochester, MN are.
