The Alliance of Community Health Plans (ACHP) and its members recognize the importance of taking a community-wide approach to improving health. Health plans play the roles of leaders, facilitators and partners in working to improve the health and well-being of communities through addressing the social determinants of health.

Join us on Thursday, March 23 at 1:00 p.m. ET for a Twitter chat to discuss how not-for-profit health plans are addressing health equity. The Twitter chat will feature examples from UCare, Group Health Cooperative of South Central Wisconsin, HealthPartners and Dean Health Plan/SSM Health on their work in the community and explore these critical questions:

  • What role can health care organizations play in addressing the social determinants of health within communities, specifically health equity?
  • What are the keys to effective community partnerships? How do organizations build strong relationships?
  • What are some examples of organizations addressing health equity internally to better serve their communities?
  • How do you measure program success?

Join the conversation on Twitter from 1-1:45 p.m. ET Thursday, March 23 and use the hashtag #ACHPCommHealth.
