Quality Improvement Interview: Fallon Community Health Plan

Fallon Community Health Plan has established itself at the top of the charts for the past three years on the Centers for Medicare and Medicaid Services’ Medicare Advantage Star Ratings.  This year, the plan once again received 4.5 stars, an indicator of its commitment to – and success at – providing high quality health care to its members.

We spoke with Beth Foley, senior director of Quality Services at Fallon, about the health plan’s performance, including its strengths, areas of improvement and measures on which it is focusing.

Because of the plan’s history as an integrated staff model, in which preventive medicine is integral to patient care, Fallon has traditionally done very well on preventive and screening measures. A substantial portion of the health plan’s Medicare membership is seen by a provider network with which Fallon has a close working relationship.

It is due in part to this connection, along with a robust electronic medical record, that screening and preventive measures have remained a particular strength of Fallon’s. As Foley says, it is second nature to make sure members are getting their screenings. “That’s what we’re all about.”

Fallon also performed highly on the Care for Older Adults measure of the Star Ratings, pertaining to Medicare Advantage Special Needs Plan (SNP) enrollees (vulnerable populations who include dual-eligibles and those with chronic conditions).

ACHP Plans Focus on Quality

ACHP members enroll about 14% of Medicare Advantage members, but enroll 98% of beneficiaries in CMS 5-star plans (the highest rating). Of the eleven plans that received 5 stars in 2011, eight are ACHP members.

In the 2012-2013 NCQA Health Plan Rankings, all of the nation’s top ten Medicare plans, seven of the top ten plans in the commercial rankings and six of the top ten plans in the Medicaid rankings are offered by ACHP members.

Health Plan Innovations Transform Primary Care to Better Serve Patients

The Alliance of Community Health Plans (ACHP) has released a new report, “Strengthening Primary Care for Patients,” describing the efforts undertaken by 17 ACHP member organizations to improve the health care system by transforming primary care. The report focuses on key components of primary care redesign and the potential for improved patient care. It is the third publication in ACHP’s “Health Plan Innovations in Patient-Centered Care” series, which highlights the essential role health plans play in improving care quality, enhancing patient experience and reducing health care costs.

Michael W. Cropp, M.D., of Independent Health Appointed 2013-2015 ACHP Board of Directors Chair

The Alliance of Community Health Plans (ACHP) announced today that Michael W. Cropp, M.D., president and CEO of Independent Health in Buffalo, N.Y., has been elected by his colleagues as the new chair of the organization’s Board of Directors. Dr. Cropp succeeds Scott Armstrong of Group Health Cooperative of Seattle, Washington, who served as chair of ACHP’s Board of Directors for the last two years.

ACHP Report Details Initiatives to Reduce Prescription Painkiller Misuse, Improve Patient Health

Community-based health plans are taking a variety of approaches to reduce the misuse of prescription painkillers while assuring patients in pain get the relief they need, according to a report released today by the Alliance of Community Health Plans (ACHP). The report, “Ensuring Safe and Appropriate Prescription Painkiller Use: The Important Role of Community Health Plans,” profiles 16 ACHP member plans that are working closely with community-based organizations in states across the country.
