The Alliance of Community Health Plans Foundation recently completed work on a major multi-year Centers for Disease Control & Prevention project that coordinated clinical intervention sites to help alleviate asthma among children in America's inner-cities.  The tool kit that appears below contains useful lessons from the project and is a must-read for health care organizations that are considering establishing their own interventions to help Inner-city children and families control childhood asthma.
