Priority Health has integrated the price of prescription drugs into its Cost Estimator tool, as part of a continued effort to help members understand their health care costs. More than 6,000 prescriptions have been added to Cost Estimator. Usage of the tool has steadily increased to 10 percent of commercial members.
By coordinating teams of case managers, nurses, social workers, paramedics, therapists and aides, Priority Health’s Tandem365 partnership has been able to reduce ER visits by 52 percent. The teams help seniors with medication, meal prep, shopping and transportation to a doctor’s office to curb ER and hospital visits.
