UCare and Federally Qualified Health Center Urban Health Network Model Selected as an Accountable Community for Health

UCare announced that its partnership with the Federally Qualified Health Center Urban Health Network (FUHN) is one of 12 Accountable Communities for Health (ACH) to receive grant funds that will support efforts to promote health and improve health care by strengthening clinical and community partnerships in Minnesota. UCare/FUHN will receive a $370,000 grant through the ACH program as part of the Minnesota Accountable Health Model. This two-year grant is the maximum amount available from the $4,440,000 allocated by Minnesota’s Department of Health and Department of Human Services for ACH activities.
