Capital District Physicians’ Health Plan (CDPHP) is partnering with Trinity Alliance of the Capital Region and the Commission on Economic Opportunity to launch The Community Health Project. Trained community health professionals are reaching out to CDPHP members who have been identified as not maximizing their health benefits.
This free service will provide information on issues including caring for children with asthma, eating well on a budget and managing chronic diseases. In addition, community health workers assist individuals with accessing medical care through a primary care doctor. The goal of this initiative is to educate and empower individuals and families to help them identify and achieve their health goals.
