Patients Get Health Care at Home

UPMC and Geisinger Health Plan are employing innovative approaches such as “remote monitoring” to keep people out of the hospital. The program is designed to create a less stressful care environment by allowing patients to stay in their own home and aims to reduce trips to the ER. Plans have found the plan successful: Geisinger Health Plan found a 23 percent lower hospitalization rate in heart failure patients using remote monitoring – and savings of about $216 per heart failure patient per month between 2008 and 2012.
