Outreach, Monitoring Help Patients with Depression Plus Heart Disease, Diabetes

HealthPartners is part of a Medicare program developing a new way to treat patients with heart disease, diabetes and depression. The approach, called COMPASS, involves care managers, who motivate patients, as well as weekly team meetings to discuss an individual’s progress. HealthPartners and the Institute for Clinical Systems Improvement are tracking the outcomes of the program.
