How Effective Can Remote Monitoring be at Reducing Readmissions?

The Institute for Healthcare Improvement (IHI) has compiled a compendium of 15 promising interventions to reduce readmissions. Among the approaches are proactive monitoring, home visits, telephone consultations, telehealth and telemonitoring. Remote monitoring brings together all these elements and, as a result, can have a significant effect on reducing readmission rates.

In its pilot program, Fallon Health is integrating remote monitoring and case management systems and increases identification of the early onset of chronic disease episodes of seniors for more effective care coordination.
