Community Paramedic Home Visit Pilot Program Begins at Regions Hospital

A pilot program that helps reduce emergency calls and unnecessary hospital readmissions is now expanding through a partnership between Regions Hospital and the Saint Paul Fire Department in Minnesota. The collaboration will help provide home visits from specially trained paramedics for patients who have recently left the hospital, focusing mainly on patients with congestive heart failure, which has one of the highest readmission rates.

As part of the discharge process, physicians will refer patients with congestive heart failure to the Saint Paul Fire Department for a home wellness check. There is no cost to patients or health plans for the visit.
