CDPHP Engages Local Doctors in Health Care Reform

Seventy-five primary care primary care practices from the Capital Region and Hudson Valley were selected by the Centers for Medicare and Medicaid Services to participate in the Comprehensive Primary Care (CPC) initiative, a public-private partnership to improve the quality and efficiency of health care.

CDPHP is currently contracted with 48 of the 75 practices selected to participate in the CPC initiative. On April 1, 2013, CDPHP expects all 48 contracted practices to be fully engaged in the process of practice transformation.
