Care At Home: Meeting Patients Where They Are

To bring care to where people are – and where they prefer to be – Kaiser Permanente Northwest has established Primary Care @ Home and Mobile Health Partners.The Primary Care @ Home pilot program began in response to patients in care homes who were having difficulty accessing outpatient primary care and suffering high rates of emergency department and hospital use. Under the program, clinicians would visit patients in their homes. The program has since been expanded to include many types of residential facilities and the care team has expanded to include nurse care practitioners, physician assistants and clinical pharmacists, among others.

Mobile Health Partners is a home-based care program in which paramedics, under their scope of practice, administer some medicines, draw blood for lab tests and take EKGs, among other services performed.
