Community Systems of Care: Accountable Care Organizations in the Context of the Triple Aim
The overall mission of ACHP member organizations is to drive Triple Aim outcomes in their communities. Most ACHP member plans believe that achieving this mission requires the engagement and cooperation of other community stakeholders that are jointly accountable for the quality, cost and patient experience of the population served. A powerful general framework for driving toward this model is the ACO; however, the specifics of how the ACO would be implemented are unclear, and the promise of delivering results is unverified.
ACHP has partnered with the Institute for Healthcare Improvement (IHI) to promote awareness and further best practices with regards to health care delivery at the community level that is both accountable and sustainable. To that end, ACHP has developed the following goals:
- Create a forum for ACHP member organizations and IHI Triple Aim participants who are interested in creating community systems of care and learning more about Accountable Care Organizations
- Provide participants with a better understanding of the ACO movement nationally and where there may be potential for engagement
- Help participants identify what are the next steps they could take to drive their communities toward Triple Aim outcomes in the context of creating Community Systems of Care
