The latest installment in the ACHP Innovation Profiles series focuses on how Capital District Physicians’ Health Plan (CDPHP) is working with local organizations and businesses to improve the health of its community.
In 2013, CDPHP partnered with Hannaford Supermarkets and the Albany YMCA to create the Healthy Living Center — a place for exercise, health education and nutrition improvement — inside of a Hannaford supermarket. It offers fitness classes, health care advice and healthy eating tips, and all of its programs are free of charge. The center has had more than 1,000 visitors since its opening in October 2013, and 800 people continue to visit the center on a regular basis.
Through the power of partnerships, CDPHP is extending its reach of care beyond the clinical setting and offering its community a roadmap to a healthier life.
