Practice Transformation Subcommittee - Patient-Centered Medical Home Collaborative Web Conference

This Subcommittee of the Patient-Centered Medical Home Collaborative was formed to help define the necessary processes of transforming practices into medical homes. On Wednesday, July 8, the Subcommittee will discuss integrating patient-specific information into the improvement process. The webinar will feature a short presentation by Beth Waterman, vice presient of Health Improvement and Care Innovation, who will present HealthPartners' use of the 10 Case Review tool.
