ACHP Framework for Community Health Work
ACHP plans recognize the importance of taking a community-wide approach to improving health, and view their role as leaders, facilitators and partners working to improve the health and well-being of communities. The drivers of a person’s total health include clinical health care, health behaviors, socio-economic factors and environmental factors. ACHP plans are committed to addressing all these drivers as part of the movement toward building healthier communities. Our plans are developing innovative programs and partnering with other community leaders and organizations to extend the reach of health care beyond the clinical setting and into daily lives.
ACHP has written a series of brief profiles describing ACHP member initiatives that improve community health. We have customized a framework of the total drivers of health. Within this framework, you may access profiles by the type of total health driver they address.
