ACHP Public Webinar — Successful Strategies for Adopting Value-Based Payment

- Capital District Physicians’ Health Plan (Albany, New York) reduced overall costs by 2.9 percent, while simultaneously increasing payments to physicians.
- UPMC Health Plan (Pittsburgh, Pennsylvania) worked with hospitals and physicians to measure how care for patients with diabetes improved their overall health by tracking indicators like blood pressure instead of focusing on how many tests were performed.
- Evolent Health (Arlington, Virginia) partners are driving measurable and replicable improvements in quality and cost outcomes, including for the most complex Medicare beneficiaries. For example, Evolent Health worked with WakeMed Key Community Care (WKCC) in North Carolina to support its complex care initiative that identifies and manages high-need, high-cost patients.
